Blank State option disappears from the dropdown in Copy Alert Types

Bug Description

In Evergreen 3.1.2, using the Web client.

Steps to reproduce:
1) Administration > Local Administration > Copy Alert Types.
2) Create
3) Note that the State field is blank.
4) Click the dropdown. Note that there is a blank option.
5) Select a specific state.
6) Click the dropdown again. Note that the blank option has disappeared.

I've also attached a gif of this.

The blank option is really important, since otherwise catalogers have to apply 8 different alerts with 8 different types to ensure their message is seen regardless of an item's state. It's pretty frustrating that such an important options disappears.

Side note: it would be more intuitive if the option were called something like "Any State", rather than just being blank.

The similar approach (universal type of copy alert) would be also needed when creating copy alert at checkin or checkout, renewal screen etc.

There is a blank option in the "Type" field when creating new alert, however, if the blank option has been chosen, copy alert is not applied. If one of the predefined types has been chosen, copy alert works properly.
(Tested in Evergreen 3.3.2)

I think this can be fixed by adding a dummy option in the template, to keep a blank option available as the first choice.

<option value="">-- All States --</option>

I'll test this out and see if that fixes it.
