web client: hold notification preferences missing from hold shelf dialog

In the 3.0 XUL client checking in an item that needs to go to the hold shelf, pops up a dialog that lists the patron's name, barcode and notification preferences (phone, SMS, email).

In the 3.0 web client the dialog shows the patron's name and barcode. The web client does not list notification preferences.

The notifications preferences do appear on the printed hold slips. Some of our libraries do not print the hold slips and use the dialog to confirm how to notify the patron (i.e. if they need to call them). They have asked for the notification preferences to be included in the popup holds dialog like in the XUL.
