Web Client: hold note is not displayed/printed on Hold Shelf Slip

Hold notes added via Hold Detail View is not displayed on the Hold Slip alert or printed on Hold Shelf Slip.

Confirmed 3.0, though this may be the result of a missing field in the print template editor. The hold slip receipt template in XUL includes a macro for printed notes.

I've added code that will print on the holds slip those notes which had the "Print on Slip" checkbox checked upon creation.
