Allow searching for a patron from place holds page in embedded OPAC

In the (web) staff interface, upon performing a catalog search or viewing a record in the embedded OPAC and using the Place Hold action, it would be convenient if one could search for the patron to place the hold for in addition to scanning their barcode.

Note that rather than opening a new tab/window, we've elected to display the patron search form in a modal instead.
