Bug Description

Somewhere along the way, the file js/dojo/dojo/openils_dojo.js was removed, but several references to it still exist in HTML files, resulting in 404 and burning up valuable milliseconds when loading already slow-to-load interfaces.

Found while investigating causes for bug 985295.
