Bug Description
Currently a user gets a phone call no matter what but my users want to get one or the other.
It would easy and clean if those check boxes functioned as either or and the patron can chose how they get notified.

Hmm, what version of Evergreen are you working with when you encounter this problem?
As far as I know, using the checkboxes will alter the user's default patron notification choices, but the user may still have the option of selecting their preference during the individual hold placement in the catalog. There are some situations where this does not always occur as expected, such as if the holds were created via the kid's catalog, or if staff members placed the hold on behalf of the patrons and their defaults were not configured.
Marking this bug ticket incomplete pending further details on the conditions being described, but this may be a support issue with the implementation and not a bug with Evergreen.
