Allow report creators to add documentation links to templates

Bill Erickson and I are working on new features in the reporting infrastructure to assist template creators and report writers by providing inline documentation fields and a template-level (possibly external) URL for larger documentation.

Pushed a fix (same repo) for hiding the documentation URL row in the reports editor when no URL is present. This was necessary because the URL from a previous report could still be living in the DOM and visible to the user.
