Patron self-reg needs auto-refresh and other small improvements

Further testing has shown the TPAC-based patron self-registration could use a few minor fixes and enhancements.

1. The page should automatically reload after a configured period of time to clear out any personal information left un-submitted.
2. The opac.username_regex setting should be enforced
3. Provide a helpful message for the formatting of the date of birth field.
4. Put a copy of the 'request library card' link somewhere more prominent on the login page?

Code for 1, 2, and 3 done. Branch to follow shortly.

I've pushed two additional changes:

1. double-click on a row in the pending patrons UI opens the patron in a new tab, same as the Load button.
2. Change the "Row Date" IDL label to "Create Date" for staff client consistency

I think this is ready for testing/pulling.
