failed instances do not release addresses and network indexes in some cases

When an instance transitions from PENDING->{SHUTTING_DOWN,TERMINATED} addresses and network indexes are not always released. This transition can occur when an instance:
- fails during the late stages of run instances
- fails during to boot on the NC for any reason
- if the NC is rebooted.

This manifests as run instances reporting not enough resources prematurely.
