fails to build with Qt 5.11

Bug Description

EqualX (like many other programs) fails to build with Qt 5.11, with a message like

  src/DialogPreferences.cpp:81:36: error: invalid use of incomplete type 'class QAction'

We're seeing this failure in Debian unstable, see https://bugs.debian.org/907227 .

As far as I know, this is because some includes inside Qt headers were changed to forward declarations.

I have created a patch to #include <QAction> that seems to resolve this issue.
