Mouse cursor does not scale on HiDPI

Bug Description

When you set scaling to 2 pretty much all elements scale to 2x their original size (sometimes pixelated) except the mouse cursor, no matter which cursor theme you choose.

I can confirm this but only sometimes. It appears to use the correct cursor size only when hovering over certain things. It's very strange. Files icon view? Normal cursor. Files sidebar, tiny cursor.

When the cursor is in the client area of a native Gtk+ 2/3 app (including the Gala window manager) and is the normal state (not the hand or `I`), it isn't scaled up. Otherwise like in a Qt or Chromium based app, it's scaled up.

An except is in the icon view of Pantheon Files, the cursor is scaled up but wrong -- what I set is `Elementary`, but `Breeze` is shown.

I see this on ElementaryOS 0.4 release with updates as of 2017-01-15 and have been for a while.

In my case, my Dell XPS 13 9360 has a 13.3" 3200x1800, so the high-dpi resizing works fine on everything. When plugging an external monitor, it removes the high-dpi resizing on everything except certain apps, eg gedit.
