Icons with the same name appear differently at different sizes

Bug Description
For instance, 16/go-next is a completely different style of icon than 48/go-next.
This causes applications on high pixel density screens to use the wrong icon. See the attachment for a screenshot of Midori on my HDPI display to see what I mean.
Another instance this problem appears is for the view-refresh icon in Chrome. For some reason Chrome expects the 16px icon and it looks bad since the style doesn't match the others.

We'd prefer if you re-file this as individual bugs like "arrows inconsistent between sizes", "refresh 16 different from refresh 24", etc versus one bug like this.
