Make actions/add more consistent with other icons

Bug Description
actions/xx/add and list-add are different from address-book-new, contact-new, document-new, event-new, folder-new, tab-new, tag-new and window-new.
In my opinion, both should look like the plus signs contained in those other icons, without a circle.

I think its good that the icon explicitly shows what exactly you're doing. And in a situation where you have 2 or more of these icons in the toolbar, everything will be clear.
