Bug Description

This is somewhat related to Bug #927553, but not the same. Currently, the Feedback page shows the developer's initial submission message, comments made on the review page, and comments made on the Feedback page. But, it's not a complete log of the history of the app. For example, when a change is submitted, or a reviewer approves that change, it makes no entry on the Feedback page.

For sanity in coordinating between multiple reviewers, we'd like the Feedback page to show some (short) record of every action taken by the developer or reviewers. Any change in status, update to the files, comments made, approval/rejection/publish button clicked, etc could benefit from a timestamped entry in the Feedback page, with a record of who made the action.

It would be useful to talk at UDS about the possible actions that might be logged.

This would mean including all of the TransitionLogs in the feedback page, interleaved with the ApplicationReview objects.

The feedback page is already visually unclear, it's hard to tell where one review ends and another one starts. It would be great to get a bit of input from design, so that Reviews and Transitions can be easily distinguished.
