Add URL to queue entry for change of state notifications

Bug Description

Right now we send notifications for new apps or change of statuses to one or more mailing lists of choices, so that reviewers are notified of these changes. These e-mails include the name of the application and any feedback related to the change of status.

Upon receiving such an e-mail, reviewers then need to open a browser, match the app name to an existing app in the queue and then click on the app to start the review. The same for app authors, although for them it's easier, as the app appears in the initial myapps page, along with any other ones they might have submitted.

It would be nice if the notification e-mails would directly contain the URL to the app entry in the queue (e.g. https://myapps.developer.ubuntu.com/dev/apps/351), so that reviewers could just click on it and be taken to the app and start reviewing straight away.
