Name separation for contacts record is too strict

Bug Description

The contacts record format defines three name fields for each contact:
first, middle, last. However, this fails to account for those people whose
names don't conform to this scheme.

There will be people whose have a different number of names, who don't
have names that have an ordering in this sense, and people who have multiple
names that combined make up e.g. their family name.

Asking for these to be stored as first/middle/last encourages bad user interfaces
that don't work around the world.
