unable to open couchdb address book, Could not create 'contacts' database: Moved Permanently

When I try to open my Ubuntu One couchdb contacts address book there is a long pause of about 30 seconds then I get an error message saying "Permission denied" in Evolution.

I have tried restarting the couchdb as per recommended process and this is my E-D-S output (which is also attached).

Can you please restart desktopcouch as stated in http://www.freedesktop.org/wiki/Specifications/desktopcouch/Documentation/Troubleshooting , and then try again? Try to get the e-d-s log again, just in case, but I think it's desktopcouch not running

Yes I think you are right. When I ran the 4th step to kill and restart desktopcouch, step 4 opened Firefox with Problem Loading Page - Unable to connect Firefox can't establish a connection to the server at localhost:51628.

Here's the terminal output of restarting desktop couch:
