designate-dashboard should help users with FIP selection

Bug Description

The designate record creation screen has a empty text box for providing an IP address to go with a record.

While this is very flexible, it also leaves more simple use cases with a challenging user experience - the users must have another screen / tab where they look up their fip list and pick and choose which fip to use.

It would be nice to have a simpler option on the record creation screen where users can have a drop down of fips instead of a free form text field for ip addresses.
