I just stopped by this bug, and I noticed the patches that I sent a while back via email never made it to the bug tracker. So, I've attached them here manually. Let me know if you have any questions.

Very nice patches. I like the refactoring. And the handling of access keys. There were minor stylistic deviations which I fixed (no C++ style comments, always space before bracket).
