Bug Description

It looks like we're bucketing a number of instances against "failed":

[default@crashdb] get Buckets [utf8(failed)];
Returned 6208 results.

This is really odd. Looking at lp:daisy, we've only ever used 'failed:%s' % crash_signature for the bucket ID. I could understand 'failed:' (which doesn't exist, by the way), but 'failed' implies something else is writing into the Bucket CF. Looking at a few of these reports, they don't have a DuplicateSignature field. Some are as recent as today (d5f41b94-9cc8-11e2-a921-e4115b0f8a4a).
