thanks for your bug report. Can you please tell when did this problem started to happen for you? Did it happen after an update? Please run unity --reset in a terminal and let me know if that somehow fixes the issue for you.

The problem started to happen when final version of ubuntu 12.04 was released. Through alpha-beta2 I didn't experience everything was ok. I didn't upgrade to the final release, I just reinstalled on a new partition.

Since I experience this problem on Cinnamon, Unity 3D, gnome classic session (excluding the no effects version), and gnome 3, the problem lies in something else in Ubuntu 12.04. Maybe its xorg, compiz, or something that isn't working well with Nvidia drivers. I had version current and I'm using verison current updates.

Results from running unity --reset
When Unity 3D tries to load and manages not to have a segmentaion fault, it shows this at the end:

"WARN unity.screeneffectframebufferobject ScreenEffectFramebufferObject.ccp:170 unsupported format
WARN unity.gdk <unknown>:0 compiz: Fatal IO error 11 (Resource Temporiraly unavailable) on X server 0.

gtk-window-decorator: Fatal IO error 11 (Resource Temporiraly unavailable) on X server 0.0."

I got this from running the command in terminal on Unity 2D:
WARNING: Unity currently default profile, so switching to metacity while resetting the values

Here's what happened:
Since I have no window decorators in Unity 3D when I log in and can't switch window focus, I logged on to Unity 2D.
I opened terminal and typed in 'gtk-window-decorator --replace' and saw that that action never completed.
Next I ran 'unity --reset' to bring up Unity 3D on top of Unity 2D(This always failed in the past), I was surprised that the window decorator came back after is disappeared and I could switch window focus.
I opened ccsm and tried to enable 'GTK Loader' in Utility category. That caused my window boarders to go away and the option was still unchecked. I lost the ability to grab window focus because of that so I logged out.
I loged into Unity 3D and opened one of my bash scripts on my desktop so I could access terminal in a GUI instead the using TTY terminals. I used CTRL + SHIFT + T to open a new tab and interred in 'gtk-window-decorator --replace' then 'unity --reset' and logged out and logged back in and it was fixed. In Unity 3D the 'GTK Loader' in Utility category is still unchecked. Cinnamon and gnome classic session also work.
Unity and cinnamon crashes (not the gtk-window-decorator) and comes back within 5 seconds not very often, usually when My zram swap of 373.9 MiB nears full capacity.

Befor I logged out in Unity 3D, I tried to enable 'GTK Loader' in Utility category of ccsm again and it failed like it did when I ran unity 3D on top of 2D. After that I could change window focus so I ran in tty1 'unity --reset' and tried to copy it with 'sudo screendump 1 > ~/Desktop/1.txt' but it only copied one page:
