Allow to attach medicines to visits/patients

Bug Description

As suggested by Cody Jordan (http://launchpad.net/~jeeper74), now that we have a private medicine database it should be possible to attach these medicines to visits and/or patients.

Possibly also online medicines could be attached by automatically downlading a copy in the local database.

Some questions about the possible implementation, that I'm raising here to obtain some discussion on the topic:

1) I think that associating medicines with patients is more reasonable that associating them with visits, even if a shortcut for doing it could be present in every visit (and so in every visit you could see medicines associated with a given patient). But I may be wrong.

2) I'd like, at this point, to migrate the local medicine database inside the main data structure of Clinica, so it would be better integrated and shareable across LAN, in case of network installation of Clinica. That would require a database migration, that I'd like to do before releasing version 0.3.0

3) I think it should be possible also to associate a patient/visit with medicines found online. In that case I suggest automatically downloading the medicine and saving it in the private medicine database. Need to think about a reliable way to avoid duplication of entries.

4) What would be a reasonable way to associate a medicine to a patient? I would say that you could choose a medicine and then add a "comment" about dosage, particular indications, maybe period of interest?
