VMAX Driver Queens - FC live migration failure

Fix to send down the correct device information to os-brick for FC,
for the corresponding host. Also, a fix to remove the device
from the correct masking view when the live migration process
has failed on the compute node.

This is only applicable to Queens, once multi-attach was supported
in Rocky, the live migration process was also re-architected.
