NetApp E-Series: instances can't be started again after shutdown

Bug Description

Our environment is Ubuntu 16.04.
OpenStack 16.0.5.

We're using NetApp E-Series backend for Cinder with CHAP and multipath enabled.

Steps to reproduce:

Scenario #1:
- launch a new instance which boots from volume
- create new volume and attach it to the same instance (instance should have at least two volumes attached simultaneously)
- shutdown the instance
- try to start the instance

The result is as follows:
- instance cannot be started anymore
- errors appears in nova-compute.log: http://paste.openstack.org/show/643864/

Scenario #2:
- launch a new instance, let's call it "A" (settings as above - create new volume and boot from)
- launch new instance "B" (settings as above)
- stop instance "A"
- try to start the instance "A"

The result is same as in scenario #1.
