initialize_connection eqlx issue

The eqlx driver has potential problems with nova live migration.

Nova's live migration calls cinder initialize_connection multiple times for the purposes of collecting the connection_info for a volume. Driver's need to be safe in initialize_connection and not assume that it's being called to create a new export on the array to the initiator.

if the access already exists it might fail? (it always executes the command to create access rights)
