xiv_ds8k driver fails chap support due to missing parameter

CHAP encryption is supported in the xiv_ds8k cinder driver, but the value specified in the cinder conf file is not passed and the feature does not work.

The fix is minimal (patch attached).

    Adds xiv_chap to xiv/ds8k driver configuration

    In order to support CHAP, xiv_chap was added to
    the driver configuration. Valid values are disabled
    and enabled (default: disabled)
