Generic iSCSI copy volume<->image doesn't disconnect

Bug Description

The code in driver.py that implements copy_volume_to_image() and copy_image_to_volume() for the base iSCSI class doesn't disconnect from the iSCSI target. This is because there is no indication of whether or not it is safe to disconnect from the target (other LUNs of the same target may be in use). This leaves around stale connections.
