Bug Description
We need to update each project that is a part of checkbox-project (stack) to have a short and useful bug report information template that gets displayed on the bug report page.
The information must include:
1) A link to the longer version (readthedocs)
2) A command to run to attach versions of all the packages
3) A command to run to attach names and versions of all the providers.

Should we instead add a task for each project? This bug has been here for a year with no action which makes me think this is a poor place for it to exist. I'll set to incomplete pending a decision.
