drop support for use-ceph-optimised-packages

Purhaps the use-ceph-optimised-packages config option should be documented as deprecated?

The apache repo dir is gone from http://gitbuilder.ceph.com and the libapache-mod-fastcgi dirs are for precise and trusty only.

The preferred way to use ceph-radosgw is to use the embedded http server these days; we should drop all remains of the patched up hacked up mod-fastcgi thing that used to happen.
