Using snap channel 21.04/stable results in blocked unit

Bug Description

When setting

juju config grafana snap_channel="21.04/stable" the unit goes into blocked state with message:

Invalid snap channel: 21.04/stable

but 21.04 is a valid channel https://snapcraft.io/grafana

The charm validates changes in the snap_channel parameter using these tracks [1].
