Underlying issue is:

 2018-04-04 05:06:04.583 16701 ERROR cinder.volume.manager VolumeBackendAPIException: Bad or unexpected response from the storage volume backend API: Error connecting to ceph cluster.

However I can't tell from the logs exactly why that's happening - I would suspect its network related.

James - I can't see your original bundle - can you direct attach to the bug report and detail what provider and network space configuration you are deploying with.
