OpenStack storage class provisioner update got reverted

Bug Description

The external OpenStack cloud-provider was updated to 1.17 in PR #167 but the new StorageClass provisioner was missed and subsequently added as part of the ceph-csi update in PR #168. However, the ceph-csi update was reverted in PR #171 and the OpenStack provisioner change was mistakenly included in the revert.

Looks like we need to tweak this just a bit. The upstream v1.17.0 tag is 1 crucial commit behind the upstream release-1.17 branch. We'll need to follow the branch for now:
