Deleting an application doesn't ask for confirmation

Bug Description
When I attempt to delete an email or device from my account, I am asked for confirmation. However, when I delete an auth'd application, it just deletes it without confirming. We should ask the user to confirm the deletion, same as with other parts of the SSO interface.

When we do this work, we should also change the buttons to use <a href="..." class=" btn-sm" ><span> Delete< /span>< /a> links which will then pick up the consistent styling with other parts of the interface (from bug #948995).
