Bug Description
There is no 2F Auth item in the 'login support form' - we should add a 2F Authentication form to the list.

This is a difficult one. I've discussed with IS and they would prefer not to have the staff helpline number on a publicly visible page (it needs to be public because a user may be unable to log in due to a lost/stolen 2f device. This also prevents us linking to any other public page with the number for the same reason. We'll need to consider this carefully in the internal messaging about the release of this new feature and, maybe, IS will need to review its decision if it causes a problem.

Actually ... http:// dictionary. reference. com/browse/ helpline

I agree with this so I'm changing the bug title/description to more specifically address it.
