new U1 user created email record but no account

Bug Description
A user just reported rather odd behavior. They created a new account through U1 and SSO somehow created only part of the account. I see a record for the email address but no actual account is associated with it. The original report was:

I could not access to ubuntu one. I have registered and received an email. When I clicked the confirmation to activate and continue, there was an error. I tried to use the link to recover my password also did not help. No confirmation sent to my email. Could you please help me to get access to ubuntu one.

To allow the user to try again without deleting the broken record, I modified the email address to be '<email address hidden>'.
What sort of situation could result in a partially-created account?

I cannot think of anything out of my head. If we had an oops id this would shed some light on the issue (but I hear we cannot get it now).
Can we know if this account was created via the web or desktop ui?
With that we could try to locate the account in the logs, and see if there is anything there.
