The following forms are gone https://forms.canonical.com/sso-support/

Replace with the FAQ: https://help.ubuntu.com/community/SSO/FAQs

Places I see it used:
1. https://login.ubuntu.com/+bad-token
2. and Support on the bottom of every page on login.ubuntu.com/
