Separate several footnote/endnote references

It's possible in Microsoft Word to link several footnotes/endnotes to one word.

As these are sequentially assigned a number, for example:
footnote 1 as 11
footnote 2 as 12
endnote 3 as 13
the resulting output has footnote reference displayed as 111213 without any separators.

Please add some separator between these. I'd suggest simple space (or maybe  ), anything else would just make readability worse.

Fixed in branch master. The fix will be in the next release. calibre is usually released every alternate Friday.
