A ToC item is lost when converting from FB2 to EPUB

Convert the attached file, "book1.fb2", to the EPUB format. Note that the "Annotation" Table of Contents item is missing in the EPUB book's ToC.

Calibre version: 6.27,
OS: Windows 10 Home.

FWIW, the "Annotation" ToC item is also lost when converting to MOBI, AZW3 and PDF.

Fixed in branch master. The fix will be in the next release. calibre is usually released every alternate Friday.
