Bug Description

1. Open the Quick Start Guide that comes with calibre in the viewer.
2. Press Ctrl+t to display the table of contents.
3. Press Ctrl+h to display the highlights pane.
4. Click on "2.1: Background" in the ToC.
5. Select "If you are only buying e-books" and press "q" to create a highlight. Note that the header for the highlight in the pane is "2.1: Background".
6. Select "from the store associated" and press "q" to create a second highlight. Note that the header for both highlights in the pane is now "Common Tasks > Task 2: Conversion > 2.1: Background".

I'm not sure what to expect here, but this behavior seems wrong.

Calibre version: 6.26,
OS: Windows 10 Home.
