Calibre doesn't load first page of CBC file as cover

Bug Description
For CBZ files, Calibre uses the first page as cover.
But when I reorganize the cbz files as a CBC file because I need a table of contents, Calibre doesn't load the first page as cover anymore.

To clarify it further, the "first page" mean the first page of the first cbz file in the CBC.
I actually tried to keep the "cover page" at the root folder, but it doesn't work.
