Ctrl-clicking on tag browser & book details ignores "match all items."

Steps to replicate:

1. In tag browser, set the search type when selecting multiple items to "match all items."
2. Click on an item in the tag browser.
3. Ctrl-click on an item in the details pane to add it to the search.

Expected results: It adds the additional item with an AND clause.
Actual result: It uses the OR clause.

Note that "match any items" uses OR as expected.

match all items applies only to the tag browser, not book details. IIRC
there is no way to use AND when clicking in the book details. I suppose
one could in theory have shift+ctrl+click do that.

Fixed in branch master. The fix will be in the next release. calibre is usually released every alternate Friday.
