Bug Description

Hello!

Calibre 5.14
SO win 8.1

Thank you very much for the update on the process of update book internal.
For those of us with older readers, it is wonderful to be able to keep the TOC, plus add the NCX.

But the option to deactivate the confirmation dialogue does not remember the option chosen, in my case keep the TOC.

It would be great to be able to select that option by default and to be able to use it from "Polish book".

I have encountered an additional problem when using this new option. When I deactivate the confirmation dialog, it does not allow me to restore the confirmation dialog again.
When I click on that option in the editor settings, it tells me that there is nothing to restore. However, if I restart the program, the dialog reappears.

And if I check again that it doesn't ask for confirmation, when I restart, the dialogue appears again.

Eternal thanks for your hard work.

Note that POlish Books is designed to run without options, that is not
going to change. However I will have the skipped dialog remember the
choice in the future, when running the tool from inside the editor.
