E-book viewer: some key combinations don't work

Bug Description
1. Open the Quick Start Guide that comes with Calibre in E-book viewer.
2. Open Preferences (Ctrl+,) select "Keyboard shortcuts", select "Toogle the toolbar" (perhaps any other action can be chosen).
3. Add the following keyboard shortcuts to the existing one, "Ctrl+F11":
"d", "Alt+d", "Ctrl+d", "Shift+d", "Alt+Ctrl+d", "Alt+Shift+d", "Ctrl+Shift+d", "Alt+Ctrl+Shift+d". The list you should see is on left side of the attached file. Click "Ok".
4. Out of all the added shortcuts only "d", "Alt+d" and "Ctrl+d" work.
This may be relevant: after adding the shortcuts, clicking "Ok" and returning to the shortcuts list, the "d"s in the non-working ones are displayed capitalized (underlined in red on the right side of attached file).

OS: Windows 10 Home 1803.
Fixed in branch master. The fix will be in the next release. calibre is usually released every alternate Friday.
