Footnotes Broken When Converting From EPUB to PDF

Starting with the new v4 Calibre, footnote links (and return links) are broken when converting from EPUB to PDF. They are fine in the EPUB, and also work in converted MOBI.

This has been reproduced in multiple e-books. A sample input and output file is attached.

The command used to build the PDF:
