epub file fails at epub check

Bug Description

calibre version 3.44
OS 10.10

If I convert the attached Word file to epub with default settings, it passes the epub check at http://validator.idpf.org/.

If I change the output options to "no default cover" i, the resulting epub file does not pass the epub check.

(I tried to also attach screenshots of the output option setting and the epub check error message, but could only attach one file, so I chose the Word file.)

Fixed in branch master. The fix will be in the next release. calibre is usually released every alternate Friday.
