Word to EPUB adds a digit at the end of some chapters

Bug Description

Calibre 3.37, MacOS 10.12.6, Word for Mac 16.16.4
I am converting a book from docx to EPUB. About half the chapters in the EPUB have a number at the end of the chapter. Sometimes, but not always, it is the number of the succeeding chapter. If not, it is a number a little lower than that of the succeeding chapter. You can see the problem at the end of chapters 8, 11, 12, 13, 15, 16, 18, 19, 20

Many thanks. Now if I produce an updated version I won't have to go
through and remove the extra numbers by hand.

One other thing I didn't send in. Word lets you mark index entries.
Calibre produces an index with links instead of page numbers, but there
are at least two things wrong with it.

1. I have an index entry in Word such as {XE "examination system:
contents of exams"} and another that is {XE "examination system: US
equivalent"}. In word that produces the following index entries:

examination system
     contents of exams, 7
     US equivalent, 9

Calibre turns it into

examination system contents of exams US equivalent

If there were five subtopics, it runs all of them together on one line
like that. One can fix that by adding a little extra code at the
beginning of the index--I expect you know better than I do how best to
do it.

2. I have an index entry such as :{XE "examination system:explanations
of" \r "ExaminationSystemEnd"}

This one is marking a range of pages rather than the single page the
entry is in. "ExaminationSystemEnd" is the bookmark for that range.
Calibre, as far as I can tell, simply links to the location of the index
entry and ignores the information specifying a range of text.

I'm not sure how you could fix that, since Calibre produces a link
instead of a page number. Is there some way that clicking on the link
could take you to the beginning of the range, with the whole range
highlighted or colored or something?

A related point, that isn't really a bug but a possibility for
improvement. Suppose I have entries for the same topic in three
different places. Word puts three page numbers after the corresponding
index entry. Calibre produces three identical entries, each with a link
to a different location. That works, but it's less attractive than a
single entry with three links. Something like:

Examination system, and, and

Where "Examination system" is the first link and the "and"s are the
other two.

If you want I can write this up and submit it through your system for
reporting bugs, but I'm not sure if that's necessary.

I should add that I'm impressed by how fast and well your system for
responding to problems works.
