Epub conversion to anything else: cover picture inserted twice

Every time I convert a book I downloaded in azw3 format (where the cover page is reported once), It ends up having TWO TIMES the cover inserted in the book (often in two different size).

Looking at the source of the epub I noticed there is actually a line to display what it was reported as a cover into previous metadata, but it's just a hint, and I might be wrong.

Anyway, this happens when converting from azw3 to Epub, Docx and other formats, but it seems to me it does not happen when converting to KFX format through the related plugin.

This behaviour appeared in Calibre version 2.* and it carried on through version 3.*
I am running on Windows 10.
The above happens with any book
