Duplicate ID error after converting from DOCX to EPUB

Bug Description

After converting a DOCX file to EPUB I frequently get an error which prevents an epubcheck being passed. Running the calibre editor tool 'Check book' I get errors such as:
Duplicate id: toc_id_84 [index_split_085.html] Error index_split_085.html (line 11) The id toc_id_84 is present on more than one element in index_split_085.html. This is not allowed. Remove the id from all but one of the elements.
The offending line in this case would read:
<div id="toc_id_84" style="height:0pt"></div><h1 id="toc_id_84" class="block_14">
It's easily fixed if you understand HTML but might be challenging to those not so familiar with it.

It's not clear if this is a Word problem or the calibre conversion's problem. Sometimes I get one of these errors. at other times I get several.

I'm using the latest version of Word (Office 365). This error has been showing for a long time though.

Here's an example file which causes the error.

Fixed in branch master. The fix will be in the next release. calibre is usually released every Friday.
