Calibre can't extract cbz/cbr cover with __macosx folder

Bug Description

I noticed Calibre 2.49 can't generate cover from CBZ/CBR with __MACOSX folder in them.
I test with three different files with same result.
However, editing these files with WinRAR to delete the __MACOSX folder solve the issue each time.
My guess is Calibre has some difficulties to handle a folder within an archive starting with double underscore.

Calibre 2.49 x64

Two files in following zip.
I removed all images except the cover, with same result.
First is with __Macosx folder = not cover at import time
Second is same without = cover at import time

The __MACOSX folder contains a broken image file. When extracting covers
from comic files, calibre uses the (alphabetically) first image file --
in this case the broken image file.

I'll look into having calibre ignore files in the __MACOSX folder, but
really, having it in the archive is a bug in the comic file.
