Some theme icons aren't substituted

Hello,

If you using a theme, the icon of Calibre is substituted by icons of the theme. But for the text format icon in "Edit metadata" windows on comment part. Some of this icon isn't substitute and is the default Calibre icon display. It is the same bug in Edit book Windows (ebook-edit.exe).

The list of image not substitute in "Edit metadata": edit-clear.png, format-fill-color.png, format-text-color.png, format-text-heading.png.

The list of image not substitute in "Edit book": view-image.png, insert-link.png, code.png, format-text-bold.png, format-text-italic.png, format-text-strikethrough.png, format-text-subscript.png, format-text-superscript.png, format-fill-color.png, format-text-color.png, format-justify-left.png, format-justify-fill.png, format-justify-right.png, format-justify-center.png.

Fixed in branch master. The fix will be in the next release. calibre is usually released every Friday.
